If you have access to a printer, you can click and download the form, print out and fill in all the information, sign the form, and then use a smartphone camera to take a picture of the form and send it to info@stretchforstrays@gmail.com Or, you may use the faster Online Adoption Contract.

Stretch for Strays is not responsible for the cost to any veterinary treatment, procedure or medication subsequent to this adoption. It is possible that this pet has been exposed to canine or feline diseases that may not manifest for several weeks after adoption. Take your new pet to the veterinarian of your choice within 72 hours of adoption to verify your pet's health. Any additional vaccinations or care should be discussed with your veterinarian, but will be at your sole cost.
No Liability: Stretch for Strays, Inc., is not liable to you or any other party for any losses, injuries, damages, cost, expenses, liabilities, law suits or judgements whatsoever in connection with your adoption or ownership of the pet you have adopted.
THE ADOPTION FEE WILL NOT BE REFUNDED, EXCEPT FOR MEDICAL REASONS IF THE OWNER NOTIFIES THE RESCUE WITHIN 72 HOURS OF THE ADOPTION.
